The Individualized Family
Services Plan (IFSP) identifies the services that the child
with a disability (birth through to the child’s third
birthday) and their families will receive. The plan is based
on the child’s developmental needs and outlines specific
outcomes for the child. A difference between the IFSP and
the Individualized Education Plan (IEP) is that the IFSP
also provides for services and supports for families. The
IFSP must contain:
- The child’s present level of
development.
- The family’s resources, priorities
and concerns as they relate to their child with a
disability.
- The major outcomes for the child and
their family.
- The early
intervention services which
will be provided.
- The natural
environments in which the
services will be provided, and if they will not be
provided in these environments, why they will not.
- The dates the services will begin
- The family’s service coordinator
- The child’s plan to transition to
pre-school services
