LOGO - All About Community

The Individualized Education Plan (IEP) defines the appropriate education for the child with a disability from age three through to their 22nd birthday. It is based on the child’s non-discriminatory evaluation and is designed by the IEP team.  The IEP must include:

  • The student’s present level of educational performance.
  • Measurable annual goals and short term objectives
  • The specially designed instruction and related services that will be provided to the student.
  • The extent to which, if any, the student will not participate in the general curriculum.
  • Modifications to state/district assessments so the student can participate or a statement of why it is not appropriate.
  • Dates for the beginning services and program modifications, frequency, and duration.
  • How the student’s progress will be measured and how the student's parents will be informed of their child’s progress. This must occur at least as frequently as the parents of non-disabled students are notified.
