Eligibility
To be eligible for Mental Retardation Services you must:

Have an IQ of 70 or below

Have significant limitations in two areas, such as communication, self care, home living, and social skills

Have proof of mental retardation before your 22 birthday
-
Be a legal resident of the U.S.
What to bring
When the individual requesting services and his/her family member or friend call to arrange an interview, you will be asked to bring:
Your first meeting can take place in your home, at our office or a convenient location in your neighborhood.
When you meet with the registration unit
You will meet at least two times with a Mental Retardation Services (MRS) Registration Specialist who will determine your eligibility for MR services. If you are not receiving Medical Assistance and are eligible, we will help you enroll.
If you are eligible, the registration specialist will look at your needs and resources, help you begin developing a plan, discuss services and make a referral to a supports coordination agency.
He or she will complete forms including: the waiting list form (PUNS), application for waiver services, and social summary.
If all the information is complete, you will receive a letter within 45 days notifying you of your rights including the meditation and your rights including the mediation and appeal process.
You will also be given a choice of agencies for supports coordination right away. Other services are not always immediately available and you may be placed on a waiting list.
If you are not eligible, you will be helped to find other community services.
